Once I receive this form, I will get you booked and you will receive a confirmation of booking that contains the salon address and important information! I ask that you give a 48 hour notice when canceling or rescheduling your appointment. If you must cancel or reschedule within 48 hours of your appointment, there will be a 50% charge of the service for which you were scheduled. If the appointment is a same day cancellation, reschedule, or no show, payment for the full cost of the appointment is required.
